A DAMNING internal review of the actions of Territory Families leading up to the alleged rape of a two-year-old girl in Tennant Creek has revealed a litany of failures by the department.

But it maintained no individual was at fault for the incident it described as “tragic, yet unanticipated”.

“There is no evidence to suggest that if one or all providers had supported the family differently, this would have prevented the alleged rape,” the review read.

A heavily redacted version of the internal review, released to media on Friday morning, paints a picture of a deeply dysfunctional system in which “service intervention often lacks a clear objective or purpose”.

The review found child protection staff lack “sufficient expertise in the links between family violence and child protection”.

And investigations assessing more than one notification have a “high potential to lose critical information”.

It said there was unanimous agreement among service providers that the family had made “marked improvements” towards the end of last year, and the little girl was not in need of care and protection.

The review’s key recommendation was the development of a new client information system which would provide workers better access to a full family history to allow those workers to make “comprehensive assessments of child safety and risk”.

Territory Families Minister Dale Wakefield said she would not accept or reject the findings, but would defer a decision until another investigation by Children’s Commissioner Colleen Gwynne was complete.

That review is expected to be complete at the end of the month.

Ms Wakefield said if more comprehensive information was available to the child’s case workers, “it is likely they would have made different decisions”, which may have prevented the alleged rape.
