7 News reports the woman, 21, was allegedly tailgated by a man at Rainbow Beach, east of Gympie, about 11.30am, before she slowed down as she headed into a roundabout.
At that point, the man allegedly tailgating the woman pulled out a knife and started stabbing the car.
After the man drove off the woman went into cardiac arrest.
The passenger of the car then called emergency services to revive her but en route to the RBWH the woman suffered another heart attack.
The woman remains in a critical condition.
The man has been arrested and is currently assisting police with inquiries.
